LOCATION: FreeForm now has a satellite clinic within Wild Life Gym at #317-1201 Commercial Way. Our satellite clinic provides the same one-on-one care and functional rehabilitation approach. Your treatment takes place in a private room, with ample space in the gym to assess movement and access a wide range of equipment. Parking is available on-site. PLEASE ENSURE YOU ARE CHOOSING THE LOCATION YOU WISH TO ATTEND.

STAFFING UPDATES: Kathrine Stolborg will be returning from maternity leave this fall, beginning October 21st, 2025. She will be working Tuesday evenings at FreeForm’s satellite location within Wild Life Gym on Commercial Way.

DIRECT BILLING: Pacific Blue Cross, Manulife, Canada Life, Sun Life, GroupHealth, Desjardins Insurance and the Chambers of Commerce Group Insurance Plan.

FreeForm does NOT accept WorkSafeBC or ICBC/motor vehicle accident claims.

CANCELLATION POLICY: We require 24 hours notice for any cancellations. Clients who provide less than 24 hours notice, or miss their appointment, will be charged the full price of the appointment. This fee applies to all clients. Extended Health Companies will not provide coverage for missed appointments.
