LOCATION: WE ARE MOVING! FreeForm Physio & Pilates headquarters at 37738 3rd Ave will be relocating to Hope, BC in early 2026. We’ll continue seeing clients in Squamish through December 2025 and will share more details about our new Hope clinic soon. A heartfelt thank-you to our amazing Squamish clients for your ongoing support — we can’t wait to start this next chapter in Hope! We’re also happy to share that physiotherapist Kathrine Stolborg will continue providing care in Squamish from our satellite clinic within Wild Life Gym at #317-1201 Commercial Way. This location offers the same one-on-one care and functional rehabilitation approach, with a private treatment room and full gym access for movement assessment and exercise programming. Parking is available on-site. Please ensure you are selecting the correct location when booking.

FreeForm does NOT accept WorkSafeBC or ICBC/motor vehicle accident claims.
CANCELLATION POLICY: We require 24 hours notice for any cancellations. Clients who provide less than 24 hours notice, or miss their appointment, will be charged the full price of the appointment. This fee applies to all clients. Extended Health Companies will not provide coverage for missed appointments.
